Improving Esthetics with LASER Gingivoplasty: A Case Report

Abstract
Background: Excessive gingival display (gummy smile) is a common aesthetic concern among patients undergoing orthodontic treatment. When associated with altered gingival contours or gingival enlargement without periodontal attachment loss, gingivoplasty can effectively improve smile aesthetics. Diode lasers have become popular for soft-tissue recontouring because of their precision, excellent haemostasis, and favourable postoperative healing.
Case Presentation: An 18-year-old patient undergoing fixed orthodontic treatment presented with the chief complaint of excessive gingival display while smiling. Clinical examination revealed excessive gingival tissue and irregular gingival margins in the maxillary anterior region, with healthy periodontal tissues. Laser-assisted gingivoplasty was performed using an 810-nm diode laser under local anaesthesia. Gingival contours were reshaped according to aesthetic principles while preserving the supracrestal tissue attachment. The procedure was completed without sutures and with minimal intraoperative bleeding. Follow-up examinations demonstrated uneventful healing, improved gingival architecture, and enhanced smile aesthetics, with high patient satisfaction.
Conclusion: Diode laser-assisted gingivoplasty is a minimally invasive and predictable procedure for managing a gummy smile in orthodontic patients. The technique provides favourable aesthetic outcomes with minimal postoperative discomfort.
