Abstract
Background: Complete maxillary edentulism can compromise mastication, speech, facial support, aesthetics, and quality of life. Implant-supported hybrid (fixed-detachable) prostheses combine fixed implant retention with replacement of lost dental and soft-tissue contours and clinician retrievability.
Case Presentation: A 48-year-old woman with an edentulous maxilla and seven previously placed implants presented with a loose, ill-fitting maxillary denture, impaired mastication, and dissatisfaction with facial aesthetics. Clinical and radiographic assessment confirmed implant stability, adequate keratinised tissue, and sufficient inter-arch space for a hybrid prosthesis. Open-tray impression copings were attached to the multi-unit abutments, splinted with pattern resin, and recorded using polyvinyl siloxane in a custom tray. Jaw relations were registered, and the tooth arrangement was evaluated for aesthetics, phonetics, lip support, and occlusion. A metal framework was verified, and the definitive prosthesis was processed in heat-cured acrylic resin with denture teeth and pink acrylic. The prosthesis was screw-retained, access channels were sealed, and occlusion was adjusted.
Outcome: At follow-up visits up to 12 months, the implants remained clinically stable, with no reported peri-implant inflammation, radiographic bone loss, screw loosening, or prosthesis fracture. The patient reported improved mastication, speech, aesthetics, and satisfaction.
Conclusion: In this case, a seven-implant-supported hybrid prosthesis restored maxillary function and soft-tissue support while maintaining clinician retrievability and hygiene access.
